CSB Reports 53 Deaths in FY 2026 Incidents at Public Meeting

Incidents reported to the Chemical Safety Board in fiscal year 2026 have resulted in 53 deaths, according to Safety+Health Magazine. CSB Chair Steve Owens stated during a Sept. 17 public business meeting that the year has been particularly deadly compared to previous years. This figure is higher than the 36 deaths recorded in FY 2025.

Fatality Comparison and Board Remarks

Owens highlighted the increase in deaths from incidents reported since Oct. 1. Board member Sylvia Johnson described meetings with grieving family members after worker fatalities as among the most difficult responsibilities for the agency. She noted that such meetings never become routine.

Johnson stated that investigations involve names and faces rather than only sets of facts. She added that the CSB follows facts to ensure lessons extend beyond the incident facility.

Steve Klejst, executive director of investigations and recommendations, reported that the three-month moving average of events under the accidental release reporting rule stood below eight. This marks the first time the average has been below eight since December 2024. The average had reached 13 in April.

Recommendation Implementation

Owens emphasized that facility management should implement safety recommendations issued after investigations. He noted the importance of timely communication with the board during the review and implementation process, according to Safety+Health Magazine.
