CSB Notes Rise in Deaths From Reported Incidents in FY 2026

Washington incidents reported to the Chemical Safety Board in fiscal year 2026 have been particularly deadly compared with previous years, according to Safety+Health Magazine. CSB Chair Steve Owens made the statement at a Sept. 17 public business meeting. Incidents reported since Oct. 1 resulted in 53 deaths, compared with 36 in FY 2025.

Board Member Describes Family Meetings

Board member Sylvia Johnson stated that visiting grieving family members is something she carries with her during investigations of worker deaths. Those meetings rank among the most difficult responsibilities the agency holds. Johnson noted the meetings never become routine. She described how an investigation shifts from facts alone when meeting families who lost a loved one, leaving an empty place at the table. Johnson said questions only a thorough investigation can answer remain.

Johnson stated that the CSB does not investigate from a distance. The agency listens, asks difficult questions, follows the facts, and ensures lessons extend beyond the incident facility so one workplace incident could save a life at another, according to Safety+Health Magazine.

Executive Director of Investigations and Recommendations Steve Klejst reported that the three-month moving average of events under the accidental release reporting rule stood below eight for the first time since December 2024. The average reached 13 in April. Klejst said the data as reported is trending down.

Owens emphasized to facility management the need to implement board safety recommendations after investigations and to maintain communication during the process. Owens stated that while implementation takes time, timely responses to board outreach remain the best approach during review.
